Founded in 1988, PHSS is a charity based in London, Canada working on disability, health, community development.

PHSS is a non-profit organisation based in Ontario, Canada, that supports people with developmental, medical, and complex physical needs to live in their own homes and participate in their communities with family and friends rather than in institutions. Established in 1988 by a group of families and advocates, it began in London and has expanded its services across Ontario's east and west regions. Its services include person-centred planning, family-centred supports, home and personal care, day and overnight respite, residential and supportive housing, community and social activities, and intensive and long-term ventilation support. PHSS states a commitment to lifelong, no-discharge support and a philosophy of preserving what is sacred for each individual. The PHSS Foundation raises funds through events such as the annual Bowl-a-Thon, a golf tournament, and an online auction, and accepts donations and sponsorships. The organisation also engages in research and quality improvement.
